FordMed Analytics provides independent medical record review, healthcare data analysis, medical chronologies, documentation review, and related analytical services.

Our work is focused on organizing and analyzing information contained within healthcare records and other materials provided to us. We help clients better understand what the documentation contains, identify patterns or inconsistencies within the records, and present information in a clear, organized format.

Who can use FordMed Analytics?

Our services may be useful to attorneys, law firms, healthcare organizations, businesses, individuals, and other professionals who need assistance reviewing or organizing healthcare documentation and data.

Do I need an attorney to work with FordMed Analytics?

No. An attorney is not required to retain FordMed Analytics for our analytical services.

However, FordMed Analytics is not a law firm and does not provide legal advice or legal representation. Individuals seeking legal advice should consult a licensed attorney.

Does FordMed Analytics provide legal advice?

No.

FordMed Analytics does not provide legal advice, legal opinions, legal strategy, legal representation, or services that constitute the practice of law.

Does FordMed Analytics provide medical advice or diagnoses?

FordMed Analytics does not diagnose medical conditions, prescribe or recommend treatment, provide medical advice, or establish a provider-patient relationship.

Our services involve analysis and organization of existing healthcare documentation and data. Clinical decisions remain the responsibility of appropriately licensed healthcare professionals.

Does FordMed Analytics determine whether medical malpractice occurred?

No.

FordMed Analytics does not make legal determinations regarding negligence, malpractice, liability, causation, damages, or other legal claims.

We may identify and organize information contained within medical documentation that may be relevant to a client’s review. Determinations regarding legal claims are outside the scope of our services.

Can FordMed Analytics provide an opinion about whether a healthcare provider made a mistake?

Our role is to analyze and report what the documentation shows, not to make legal or clinical determinations regarding whether a provider acted appropriately.

Where appropriate, our work may identify documented discrepancies, inconsistencies, missing information, chronology issues, or other areas that warrant further review by the client’s attorney or qualified healthcare professional.

Can FordMed Analytics serve as an expert witness?

FordMed Analytics does not automatically provide expert-witness opinions simply because we perform record review or data analysis.

Any request for testimony, expert opinions, affidavits, or other specialized services will be evaluated separately based on the specific engagement, qualifications required, applicable rules, and scope of the requested work.

Can you tell me what my medical records mean?

We can help organize and explain information contained within medical documentation in a descriptive and analytical context.

We cannot provide a medical diagnosis, treatment recommendation, or individualized medical advice. Questions about your health, diagnosis, treatment, or prognosis should be directed to your licensed healthcare provider.

What types of records can you review?

Depending on the engagement, records may include:

Can you obtain my medical records for me?

Record retrieval is not automatically included in every engagement.

If record retrieval or assistance obtaining documentation is requested, the availability and scope of that service will be discussed separately.

Clients remain responsible for providing appropriate authorization and ensuring that FordMed Analytics has lawful access to records supplied for review.

How is my information protected?

FordMed Analytics treats client information and healthcare documentation as confidential and limits access and use to the purposes of the engagement, subject to applicable law and the terms of the applicable service agreement.

Clients should not submit sensitive medical records or personally identifiable information through an unsecured communication method unless specifically instructed to do so.

What happens to my records after the project is completed?

Records and project materials are handled according to the applicable engagement terms and document-retention practices.

Clients may request information regarding applicable retention and destruction procedures before providing records.

Do you guarantee a particular outcome?

No.

FordMed Analytics does not guarantee that a review will identify a particular finding, support a particular legal claim, result in a settlement, affect a lawsuit, change a medical decision, or produce any particular outcome.

Our role is to provide objective analytical services based on the information available to us.

Are your findings legal or medical opinions?

No.

Unless specifically agreed to under a separate engagement that lawfully permits such services, FordMed Analytics provides factual and analytical observations regarding the materials reviewed.

Our work should not be interpreted as a legal opinion, medical diagnosis, treatment recommendation, or determination of liability.

How are fees determined?

Fees are based on the scope, complexity, volume, and urgency of the requested services.

Hourly rates and applicable fees are provided in our Fee Schedule. Larger or complex engagements may require an initial retainer.

Can I request a custom project?
